Exceptional Compared With What? Estimand Transfer and Background-Mortality Benchmarking in the Diamond Princess Passenger Cohort

Abstract / 要旨

Abstract Background

The Diamond Princess became an influential early source for SARS-CoV-2 case- and infection-fatality estimates because it provided an unusually well-enumerated, older cohort. Those analyses addressed infection denominators, delay to death and age adjustment. We asked a different question: when the reported death count is interpreted at the passenger-population level, is age- and time-expected background mortality numerically negligible?

Methods

We conducted a descriptive benchmark analysis of 2,666 passengers. Expected all-cause deaths were calculated from official pre-pandemic life tables using passenger age structure and observation windows. The primary window was 3 February–15 April 2020 (72 days). The calculation used Japan 2019 mortality, 45% male/55% female allocation within each 10-year age band, mean single-year mortality within bands and no healthy-traveller reduction. Sex-specific annual probabilities were converted to interval risks before weighting. The reference expectation was compared with 13 passenger deaths reported in Japan as outbreak-associated/COVID-related; a sensitivity outcome added one death after Australian repatriation. Alternative start dates, national life tables, sex allocations, within-band age assumptions and healthy-traveller stress tests were examined.

Results

The primary calculation yielded 8.33 expected background deaths. The background-mortality benchmark ratio was 1.56 for 13 reported deaths and 1.68 for 14. With demographics fixed, four national life tables yielded 8.33–12.41 expected deaths and ratios of 1.05–1.56. Japan-only all-female and all-male structural bounds yielded 5.64 and 11.62 expected deaths. Across aligned 70-, 72- and 86-day anchors, ratios for 13 deaths ranged from 1.31 to 1.61.

Conclusions

Background mortality was not numerically negligible relative to the reported Diamond Princess death count. This does not reclassify deaths or estimate excess mortality. It identifies an estimand-transfer problem: the same outbreak death count can inform both infection-fatality and population-mortality questions, but these require different reference structures.

要旨 背景

ダイヤモンド・プリンセス号は、比較的高齢で、かつ人数が例外的によく把握されたコホートを提供したため、SARS-CoV-2の症例致死率および感染致死率を推定する初期の重要なデータ源となった。これらの分析は、感染者分母、死亡までの遅れ、年齢調整を扱っていた。本研究は別の問いを設定した。すなわち、報告された死亡数を乗客集団レベルの死亡として解釈するとき、年齢と観察期間から期待される背景死亡は、数値的に無視できるほど小さいのか、という問いである。

方法

乗客2,666人を対象とする記述的ベンチマーク分析を行った。公的なパンデミック前生命表を用い、乗客の年齢構成と観察期間から期待全死因死亡数を算出した。主解析期間は2020年2月3日から4月15日までの72日間とした。中心計算には日本の2019年死亡率を用い、各10歳階級内で男性45%・女性55%を割り当て、階級内の単年齢死亡率の平均を用い、健康旅行者効果による低減は行わなかった。男女別の年間死亡確率は、加重前に観察期間リスクへ変換した。参照期待値は、日本でアウトブレイク関連/COVID関連として報告された乗客死亡13人と比較し、感度分析ではオーストラリア帰国後の死亡1人を加えた14人も用いた。開始日、参照生命表、性別配分、年齢階級内の年齢仮定、健康旅行者ストレステストを変更した感度分析を行った。

結果

主解析では、期待背景死亡数は8.33人であった。背景死亡ベンチマーク比は、報告死亡13人に対して1.56、14人に対して1.68であった。人口構成を固定して4か国の生命表を用いると、期待死亡数は8.33~12.41人、比は1.05~1.56となった。日本2019生命表のみで全員女性・全員男性という構造的境界を置くと、期待死亡数はそれぞれ5.64人、11.62人であった。4月15日を同一の転帰把握終点として70日、72日、86日の開始日アンカーを用いた場合、13人死亡に対する比は1.31~1.61であった。

結論

ダイヤモンド・プリンセス号で報告された死亡数に対して、背景死亡は数値的に無視できる大きさではなかった。この結果は死亡の再分類を行うものでも、超過死亡を推定するものでもない。本研究が示すのは「推定対象の移行(estimand transfer)」の問題である。同じアウトブレイク死亡数でも、感染致死率を問う場合と集団死亡を問う場合とでは、必要な参照構造が異なる。
