Language Structure of Cesarean Section Indications and Its Institutional Expansion During the COVID-19 Pandemic: Emergency Guideline Elasticity Theory (EGET) -An Observational Document Analysis in Japan

Background. During the COVID-19 pandemic, Japan exhibited "institutional expansion" of cesarean section (C-section) practice—defined as both a quantitative increase in C-section application and a categorical extension of default C-section policies to populations never addressed in any national guideline, most notably COVID-19-negative close contacts. This study asks which structural features of emergency guideline language facilitated, and sustained, that expansion.

Methods. Following Bowen's (2009) document-analysis approach, the study examines all six editions of the joint COVID-19 guidelines issued by the Japan Society of Obstetrics and Gynecology and partner societies (March 2020–December 2021), hospital-issued public notices (including Internet Archive captures), national registry and NDB rate data (Shimada et al., 2025), and international guidelines (WHO, RCOG, ACOG, RANZCOG). It introduces five reproducible instruments forming Emergency Guideline Elasticity Theory (EGET): the Elasticity Coding Instrument (ECI), a three-layer Institutional Self-Description Power (ISDP) model, Institutional Constraint Mapping (ICM), and an integration of crisis policy drift (Hacker 2004; Streeck & Thelen 2005) with the implementation frameworks of Matland (1995) and Lipsky (1980).

Results. ECI scoring revealed a step-change from ECI = 0 (Versions 1–2) to ECI = 11 (Versions 3–6), concurrent with the surge in C-section rates, whereas international guidelines scored ECI ≈ 0–1 throughout. Documentary evidence from six facilities confirmed expansion reaching COVID-19-negative close contacts, while a single prefecture maintained predominantly vaginal delivery under the identical guideline environment—an internal counterfactual showing the elevated national rate was not medically inevitable.

Conclusions. The study explicitly refrains from causal claims. It provides a reproducible, observation-based theoretical platform with seven testable propositions for future hypothesis-driven and comparative research on how emergency-guideline language shapes frontline clinical practice. No patient-level data were used; all sources are publicly accessible documents and registry statistics.

背景。 COVID-19パンデミック下の日本では、帝王切開(C-section)実践の「制度的拡張」が見られた。これは、帝王切開適用の量的増加と、いかなる全国ガイドラインでも扱われていない集団——とりわけCOVID-19陰性の濃厚接触者——への原則帝王切開方針の categorical な拡大の双方として定義される。本研究は、緊急ガイドライン言語のいかなる構造的特徴がこの拡張を促進し、かつ持続させたかを問う。

方法。 Bowen(2009)の文書分析アプローチに従い、日本産科婦人科学会および関連学会が発出した合同COVID-19ガイドライン全6版(2020年3月~2021年12月)、病院発出の公的告知(Internet Archive記録を含む)、全国レジストリおよびNDBの率データ(Shimadaら, 2025)、国際ガイドライン(WHO、RCOG、ACOG、RANZCOG)を検討した。本研究は、緊急ガイドライン弾力性理論(EGET)を構成する5つの再現可能な尺度を導入する:弾力性コーディング尺度(ECI)、三層からなる制度的自己記述力(ISDP)モデル、制度的制約マッピング(ICM)、ならびにcrisis policy drift(Hacker 2004; Streeck & Thelen 2005)とMatland(1995)・Lipsky(1980)の実施枠組みの統合である。

結果。 ECI採点は、帝王切開率の急増と同時に、ECI=0(第1~2版)からECI=11(第3~6版)への段階的変化を示した一方、国際ガイドラインは一貫してECI≈0~1であった。6施設の文書的証拠がCOVID-19陰性の濃厚接触者にまで及ぶ拡張を確認した一方、ある県は同一のガイドライン環境下で主として経腟分娩を維持していた——上昇した全国率が医学的に不可避ではなかったことを示す内部反事実である。

結論。 本研究は因果的主張を明示的に控える。緊急ガイドライン言語がいかに現場の臨床実践を形づくるかに関する、将来の仮説検証的・比較的研究のための、7つの検証可能な命題を備えた、再現可能で観察に基づく理論的プラットフォームを提供する。患者レベルのデータは一切使用しておらず、すべての資料は公開された文書とレジストリ統計である。
